Please complete the following questions in sections I through IX to the best of your ability and sign where indicated on pages 1 and 5.  The purpose of this questionnaire is to help the Attorney to determine various alternatives available to you. 

ALL INFORMATION YOU PROVIDE WILL BE KEPT CONFIDENTIAL.  Please be sure to provide ACCURATE and TRUTHFUL information, as the Attorney’s advice to you will be based upon your answers.  Many of these questions may seem personal; however, the CIS, the Labor Department or the Department of State will ask you the same questions.

After completion, please return the form to our office by email, fax, and mail, or drop it off at the location. Once we receive your completed questionnaire we will contact you to schedule your consultation.

The consultation is usually scheduled no earlier than 72 hours after the submission, in order to give the Attorney enough time to review your paperwork, strategize the best courses of action and identify possible problem areas.

The fee for an immigration consultation is $150.00. You may pay by cashier’s check, money order or cash.  You must pay before or at the time of consultation. Please note that, unless the consultation fee is submitted together with the intake sheet, a valid credit card number will be required to secure the time for your consultation.

Our office has a 24 hour cancellation policy and all no- shows will be billed full consultation price.  The consultation fee may be credited toward the agreed flat fee for your case, if the Attorney is retained within 30 days of the initial meeting.
